Instruction Request Form for the Graduate Campus Libraries

This form is to be submitted a minimum of two (2) weeks before instruction is needed. Please fill out the entire form and press the "Submit Request" button to send your request. A Librarian will contact you within three (3) days after your instruction request is received.
