Instructions:
This form is to be submitted a minimum of two (2) weeks before instruction is needed. If instruction is needed within one week, please contact Sally Bryant by phone at x4262. Please fill out the entire form and press the "Submit Request" button. A Librarian will contact you within three (3) days after your instruction request is received.
Note: This is a REQUEST for instruction only. You will be contacted by a librarian to confirm and schedule actual instruction.
